You have the right to a copy of your health information. Here is how to request it from The Center for Health Care Services.

How to request records

Records requests are handled by the Health Information Management team. A signed authorization is required before any information can be released — that is a federal requirement under HIPAA, not a CHCS preference.

  1. Complete a Release of Information (ROI) authorization

    The authorization has to identify what records you want, who they go to, and how long the authorization is valid.

  2. Provide valid photo identification

    This protects your information. If you are requesting records on behalf of someone else, you will also need documentation of your legal authority — guardianship, power of attorney, or personal representative status.

  3. Submit the request

    Submission channels, processing time, and any fees are set by CHCS policy and state law.

    If you believe information in your record is incorrect, you can ask for it to be amended.
